Eligibility Requirements:

1. Minimum of 10 up to maximum of 50 employees.
2. The Company must be registered with either the Securities and Exchange Commission or the Department of Trade and Industry.
3. Any person who is 18 yrs. old up to 65 years old and regularly employed by the company.
4. Legal dependents of an employee who were declared and considered eligible dependents of Philhealth.
